Effective Jan. 1, 2005, the Missouri Highways and Transportation Commission, acting by and through the Board of Trustees of the Missouri Department of Transportation and the Missouri State Highway Patrol Medical and Life Insurance Plan, hereby adopts the amended and restated Missouri Department of Transportation and Missouri State Highway Patrol Medical and Life Insurance Plan. This amended and restated plan is the basis for calculating benefits for medical care services and supplies received.
