Vendor To Bid as A Prime Contractor

A vendor who wishes to bid on projects of any size is required to have a Prequalification Contractor Questionnaire on file with MoDOT.  Page 4 of the questionnaire lists the required documents to be submitted along with the form. 

The Prequalification Contractor Questionnaire can be found at this link.   After your completed questionnaire and the required attachments are returned to us and approved, you will be qualified to bid as a prime contractor on highway projects at any dollar amount.

If you wish to bid only on projects under $2,000,000 please complete the Contractor Questionnaire form found here.  After your completed questionnaire and the required attachments are returned to us and approved, you will be qualified to bid as a prime contractor on under $2,000,000.

Any documentation needed to become a prime contractor requires original signatures.  Your questionnaire will be retained on file for one year.
