In order for a school to participate in Battle of the Belt, a Participation Agreement Form must be completed by October 1, 2012.
Please indicate if your school will be participating in the Safety Belt Check/Education Campaign contest and/or the video PSA contest. Please enter your school website address. Before submitting the Participation form, your school administration must be aware and agree with your plans to participate.

After submitting the Participation Agreement Form, a confirmation e-mail will be sent to the e-mail address indicated on the form.
Please write down the Registration Number assigned to your school. This number will be entered on all competition forms and submissions.
